The form asks you not to include medical details, precisely so that sensitive information is discussed in the office rather than sent over the internet.
Our web host also records standard technical information such as IP address, browser type, and the pages requested. This is ordinary server logging and is not used to identify you.
Information you submit is used to contact you about scheduling and to respond to your enquiry. That is all. We do not sell it, rent it, or use it for advertising.
We do not sell or trade your information. It may be handled by service providers who operate parts of this website on our behalf — such as our web host and the software that delivers form submissions to the office — and only to the extent needed to do that work.
